Patient Forms-New Patient Visit

The first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to Our Doctors, Dr. Janna McIntosh and Dr. Tara Denton, and our caring staff. We encourage questions and do our best to always deliver quality care.

Once you are ready to make an appointment, the best way to do so is by calling our office and speaking directly with a team member. If we do not answer, please leave a detailed message and we will get back with you within 24 hours. 

A helpful tip when calling to make a new appointment is to have your insurance ID or Soonercare ID ready for our team as well as a working email address. Once the appointment is made, our team will send a link directly to your email for you to complete your child's patient paperwork before you come to the appointment.

If you would prefer filling out paper patient paperwork the day of your appointment, that option is also available. Please arrive 15 minutes prior to your child's appointment to complete this paperwork.
